Sir Nigel John Dermot Neill was born in Northern Ireland in 1947 to an English mother and a father from New Zealand. His family relocated to Christchurch when he was seven years old. During his school years, he adopted the nickname "Sam" as a deliberate choice to avoid standing out among multiple students named Nigel, explaining that he believed a shorter name would make him less vulnerable to bullying in a New Zealand educational environment.

Neill's acting career began while he was studying at Canterbury University, where he performed in theatrical productions across Christchurch and Wellington. His first film appearance came in 1971, but his breakthrough role arrived six years later with Roger Donaldson's film Sleeping Dogs. His profile expanded internationally following his 1979 performance opposite Judy Davis in My Brilliant Career, which became a global success and led to his eventual relocation to Australia.

In March 2023, Neill publicly disclosed that he had been receiving chemotherapy for a year following a diagnosis of stage three angioimmunoblastic T-cell lymphoma, a rare blood cancer. At that time, he reported that the disease had entered remission but that he would require ongoing monthly chemotherapy treatments. In 2023, he published his memoir, Did I Ever Tell You This?, which documented anecdotes from his extensive acting career alongside accounts of his health journey.
